Patient Information Leaflet – Asthma Reviews

Why attend my asthma review?

Asthma is a chronic treatable condition however the death toll for asthma has not changed for many years, 1200 people die from asthma every year in the UK.

81% of our patients with asthma attended their review last year, we want to make this 100%.

We know that we can reduce the risk of illness and death related to asthma through annual reviews. Our patients with an asthma diagnosis are invited to start the process of their asthma review on our website or by completing a paper form collected from the practice.

You do need a peak flow meter to complete this review, if you do not yet have a peak flow meter a prescription for this item can be generated. We are aware some patients may not be in a position to pay the prescription charge for a peak flow meter, if you are in this situation please speak with one of our patient service team. They can book an appointment with one of our nursing team for a peak flow measurement to be taken at the practice.

Once you have completed the asthma review form you will be allocated a review phone call with one of our practice nurses with a special interest in asthma.

How will I know when my review is due?

Your review will always be due in your birthday month. If we have a mobile number on your record you will receive a reminder text message in the month before your birthday. We will then send reminders every time you request your inhalers. Please ensure you notify us if your mobile number has changed. Letters will be sent if you have declined text messages.

What will happen if I do not complete my review?

We strongly advise that you do complete your review for your own health benefits. Once you have gone 3 months past your birthday month we will change your prescription from repeat to acute. This means that we will not automatically sign off your prescription request. We will never refuse to issue your prescription but you will only be issued one of each inhaler at a time and you will need to collect the paper prescription from the front desk at our Greenhill site until your review is completed.

Finally, thank you for your understanding. We want to provide the best care for all our patients at the Westgate Practice.
